How this is calculated
Every plan is costed at your numbers, then ranked on subscription plus what Shopify takes per transaction. App spend is added to the headline figure when you give it, but it is deliberately excluded from the ranking: it is the same number on every row, so including it could only obscure which plan is actually cheaper.
The effective rate is the plan's card rate, plus Shopify's third-party surcharge when Shopify Payments is not your primary gateway. That surcharge differs per plan, which is why the gateway answer changes not just the totals but which plan wins.
